Before proceeding with reporting damage, please read the "Frequently
Asked Questions" about assistance to individuals. To
return here, use the <back> button on your browser.
THIS IS THE NEXT STEP:
- Report only damage to your PRIMARY PLACE OF RESIDENCE
(where you live, not a secondary home, vacation home, etc.).
- Submit all of the required information on the attached damage
reporting form. (Note: Raw estimates of damage
amounts are acceptable at this point).
- The damage must be the direct result of a disaster that is
serious and wide spread.
- A preliminary damage report is not a CLAIM. The figures your
submit are used to determine if there is sufficient uninsured
damage county wide to warrant a request for Federal disaster
assistance.
- Print the damage
report, fill it out and use one of the reporting options below.
Now you must report within seven days following the disaster using
one of these methods . . .
- Call in your report during business hours
(8:00 am to 5:00 pm weekdays) 360-385-9368.
- After hours leave a message at 360-385-9368.
Include a number where you can be reached during the day. A
staff member will return you call as soon as possible.
- Fax the completed damage report form to 360-385-9376.
- Mail your completed damage report form to 81
