Before proceeding, read the

disaster assistance to individuals. If you
have any questions call the Emergency Operations Center at 360-385-9368.
THIS IS THE NEXT STEP:
- Report only damage to your
PRIMARY PLACE OF RESIDENCE
(where you live, not a secondary home, vacation home, etc.).
- Submit all of the required information on the attached
damage
reporting form. (Note: Raw estimates of damage
amounts are acceptable at this point). The damage report is in
a printable .pdf format.
- The damage must be the direct result of a disaster that is
serious and wide spread.
- A preliminary damage report is not a CLAIM. The figures your
submit are used to determine if there is sufficient uninsured
damage county wide to warrant a request for Federal disaster
assistance from FEMA.
- Print the
damage
report, use one of the reporting options below.
Now you must report within seven days following the disaster using
one of these methods . . .
- Call in your report during business hours
(8:00 am to 5:00 pm weekdays) 360-385-9368. A special
reporting time may be announced. Check local news media or log
on to
www.emergency-information.blogspot.com to find out when you can
report.
- After hours leave a message at 360-385-9368.
Include a number where you can be reached during the day. A
staff member will return you call as soon as possible.
- You can fax the completed damage report form to 360-385-9376.
- Or you can mail your completed damage report form to 81
Elkins Road, Port Hadlock, WA 98339.
- To e-mail your report, fill it out on your
computer, save it to a unique file name and email your
completed damage report form (in .pdf) to
jcdem@co.jefferson.wa.us.
